I understand and confirm correct:- 

1. I am over the age of 21 and can provide ID if required to prove so

2. The decision to become a party hostess entirely my idea and I have not been coerced into doing so in any way to apply

3. To my knowledge there is no medical reason why I should not involve myself in this type of activity 

4. I understand any payment received for me attending a party is purely to cover my time and expenses accrued and not for any activities that may occur at the party

5. It is entirely my responsibility to pay and tax due from these expenses on a self employed basis 

6. I confirm that under no circumstances do I discuss what happens at these private events to any third party outside your marrage or relationship unless I have been given permission to do so in writing from greedy girls party organiser

7. If for any reason I can't attend a party that I have confirmed I would attend I will contact the organiser and let them know as soon as I possibly can
